Our medical team works side-by- side with you and your physician to create a specific plan of care to meet your needs, improve the quality of life and receive optimal care.
What is Concurrent Care?
Concurrent Care provides a strong support for patients with a life-limiting or other medical condition. Concurrent Care is not the same as hospice care, and can be used for those who continue receiving all medically necessary treatment that would often not be allowed in a traditional hospice program. This means that somebody with a serious illness can have all the benefits of comfort care and still receive life-prolonging treatments. Concurrent Care can you assist with the following:
- Symptom management, such as pain, shortness of breath, or nausea.
- Improvement in overall strength to complete activities of daily living.
- Team approach to ensure the best, well-rounded care.
How Do I Qualify?
A patient must be diagnosed with a disease that is active, progressive and irreversible that will result in a reduced life expectancy. Qualifying conditions include, but are not limited to the following:
- Congestive Heart Failure Disease (e.g., COPD) • Kidney Failure
- Alzheimer’s Disease
- Amyotrophic Lateral Sclerosis (ALS)

Concurrent Care Services
Our Concurrent Care services are medically-focused, and act as an extension of other primary and specialty care. Our team of board-certified physicians are knowledgeable in the area of palliative medicine, Concurrent Care and hospice.
Together, they lead an expert interdisciplinary team of advanced practice professionals, nurses, social workers, chaplains, and aides who specialize in caring for people facing a serious illness or life-limiting condition. Our team is actively involved in each patient’s plan of care in order to achieve the best outcome.
